We will review the information you provided and verify your insurance.  We will also contact your doctor to have them complete the authorization form required by your insurance company.

Once we received the required paperwork back from your doctor, you will receive a call from a distributor in network with your insurance. The distributor will contact you to review your insurance benefits and get your consent to send your first order of Men’s Liberty.

Please call your doctor’s office to let them know that you would like to try the Men’s Liberty, so they know that you have requested this product.
